Disclosure Form

This form should be completed if an employee feels that they have experienced or been subjected to sexual harassment, if they are completing the form on behalf of another employee, or where the concern relates to harassment by a third party, such as a guest, customer, contractor or visitor. 

The form can be filled out online or downloaded. If you download the form please pass the completed form to the Head of Centre and HR, who will review the concern and make contact with those involved to discuss the matter and determine the appropriate next steps.
